Job Summary:

Heartwood Extended Care in Tacoma, Washington is seeking a Social Services Assistant to support the Social Services Director in identifying and addressing each resident’s social, emotional, and psychological needs. The Social Services Assistant plays a key role in helping residents reach their full potential during their stay and contributes to effective discharge planning.

Essential Functions:

  • Participate in the admission process by meeting with residents and families to review policies on advance directives, resident rights, and non-discrimination
  • Conduct and document social services evaluations, including identifying resident needs and concerns
  • Coordinate and provide social work services based on resident and family input
  • Assist residents with financial planning, as needed
  • Communicate resident issues, personal needs, and transfers with families and staff
  • Collaborate with facility staff to address and resolve resident concerns
  • Ensure compliance with corporate, state, federal, and regulatory requirements
  • Pursue continuing education relevant to the role
  • Support discharge planning and transition processes
  • Maintain confidentiality of resident care and protected health information
  • Report known or suspected unauthorized disclosures of sensitive information
  • Address resident complaints and grievances; document and report resolution efforts to the Administrator per facility procedures
  • Educate residents on rights, living wills, and related topics
  • Participate in resident or group council meetings as needed, providing support and guidance
  • Work under the direction of the Social Services Director

Education, Licensure, & Experience Requirements:

  • Associate’s Degree (or higher) required
  • Must meet State certification requirements
  • 1 to 2 years of experience in geriatrics preferred

Knowledge, Skills & Abilities:

  • Strong written and verbal communication skills
  • Ability to work collaboratively with residents, families, and interdisciplinary teams
  • Compassionate, resident-focused approach
