Job Summary

• The Licensed Practical Nurse (LPN) in the Rehabilitation Unit provides nursing care within the scope of licensure to patients requiring rehabilitative support under the supervision of a Registered Nurse (RN)
• The LPN delivers quality care through collaboration with the healthcare team, supports recovery and mobility efforts, and ensures compliance with clinical standards and hospital policies

Essential Functions

• Provide direct patient care, administer medications, and perform treatments under RN supervision
• Monitor and document patient symptoms, responses, and progress
• Assist with rehabilitation therapies and activities per patient care plan
• Support pain management and educate patients/families on coping strategies
• Report patient condition changes and consult with RN regarding care needs
• Perform procedures such as specimen collection and vital signs
• Interact respectfully and professionally with patients, families, and staff
• Identify and address psychosocial, cultural, and spiritual needs of patients
• Ensure patient safety and comply with infection control practices
• Participate in care planning, patient education, and discharge preparation

• Understanding of medications and safe administration techniques
• Basic computer literacy
• Ability to prioritize, multitask, and work in a fast-paced environment
• Strong interpersonal and communication skills
• Awareness of age-specific needs and rehabilitation protocols
• Reliable attendance and ability to work assigned shifts
• Ability to assist in emergency situations and lift/move patients as needed
• Commitment to ethical standards and regulatory compliance

Education

• Graduate of an accredited practical/vocational nursing program

Licenses/Certifications

• Current state licensure as a Licensed Practical/Vocational Nurse
• Basic Life Support (BLS) certification (required)
• De-escalation/Crisis Mgmt. training (required within time frame specified in facility policy)

Experience

• Minimum six months of experience in an acute care or rehabilitation setting preferred
